I devised this questionnaire when a close friend wanted me to be around her during her birth as "support" for her hubby. Please feel free to use or edit for use with your own clients.
1. How do u relax/destress esp. during pregnancy?
2. What are your chosen comfort measures for birth?
3. When do you expect me to accompany you after the surges has started i.e immediately @ ASAP or when it starts to get intense?
4. How do u imagine the support of a birth companion would be?
5. How do u feel u would like to be supported during birth by a birth companion (who is not your hubby?)
6. According to your previous experience (if any) do u like to be touched/massaged during labor and birth?
7. How comfy are you with a birth companion observing or touching your private areas?
